Our services are offered on a sliding-scale basis: the amount you pay will depend on your income level.

The Healthcare Plan Legal Service Package starts at $200 for individuals and $350 for couples, with the final rate set based on your income level.* This does not include court filing fees; we can help you determine if you qualify to have court fees waived.

A 45-minute “final wishes” consultation with an attorney to discuss the details of your healthcare plan

Drafted Estate Planning Documents

Drafting the following estate planning documents for you based on the final wishes consultation: Oregon Advance Directive; HIPAA Authorization; Burial Instructions, Disposition of Remains;

30-minute Drafts Review

A 30-minute drafts review consultation with an attorney to review your drafts and a single round of revisions;

Meeting with the Notary Public

A 30-minute signing meeting with a Notary Public, where you provide the witnesses;

Final Signing with Your Healthcare Representative

Final signing instructions for your healthcare representative, if they are unable to attend the signing meeting in-person

Final Deliverables

Letter of instruction to healthcare representatives, electronic copies of final documents, and originals of final documents.
